I holder of Aadhaar Number, hereby give my consent to Relief Commissioner to obtain my Aadhaar no. and Biometrics/OTP for Authentication through UIDAI. The AUA has informed that my Biometrics will not be stored/shared and will be submitted to Relief Commissioner only for the purpose of Relief.

मैं आधार संख्या का धारक, राहत आयुक्त को मेरी आधार संख्या प्राप्त करने और यू०आई०डी०ए०आई के माध्यम से प्रमाणीकरण के लिए बॉयोमीट्रिक्स/ओटीपी हेतु सहमति देता/देती हूँ | ए०यू०ए ने सूचित किया है कि मेरे बॉयोमीट्रिक्स को संग्रहित/साझा नही किया जाएगा और केवल राहत अनुदान के लाभ देने के उद्देश्य से राहत आयुक्त को प्रस्तुत किया जाएगा |
